Please do read the Subscription Information 2008 document prior to completing the PLUS Renewal Form 2008-09. Do note that you will have a number of options regarding the completion of this form dependent on the version of Adobe you have. In all cases it will be possible to complete the form on screen by typing the required information into each field and then selecting the subscription options as relevant. You may then print the document to retain a copy for your records and then either email the document using the option button provided or either posting or faxing the form. Do note that the subscription service relating to the online availability of scanned images, data and reports is also included in this renewal package and you can specify different purchase order numbers to cover each part of the subscription or use just one. As in previous years you may also specify whether you wished to be invoiced in the 2007/8 financial year or in 2008/9.
